These phone numbers will connect you directly with a Patient Access Representative who can direct you to the appropriate department to schedule. Patient appointments are scheduled Monday through Friday from 9 a.m. to 5:30 p.m. Consultations with Tomah Health providers are also available during these hours.

For fax requests, download a referral form. Fill it out on your computer, then print and fax it to 608-377-8746. You’ll receive a response to your fax request within two business days.

Referral Services

During Clinic hours, you or your staff will be:
Conferenced with a scheduling specialist from the requested clinical service to schedule the patient’s appointment(s).

After Clinic hours, or if a scheduling specialist is not available, you will be asked:

  • Your contact information
  • Requested clinical service
  • Patient’s name, date of birth, and contact information
  • The timeframe in which the patient needs to be seen
  • Condition and/or diagnosis

Our scheduling specialist contacts the patient directly to schedule the appointment.
